Requirements
  • Three months to one year of experience in a related function.
  • A high school diploma or equivalent is required.
  • A valid driver's license, adequate insurance when using a personal vehicle, a safe driving record, and successful completion of a driver's screening background check are required. The employee must be at least 21 years old and have held a valid United States driver's license for at least six months.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways; lift at least 25 pounds using proper lifting techniques or a two-person lift; and assist clients with activities of daily living.
  • Ability to operate a computer and office equipment such as a calculator, copier, and printer.
  • Ability to speak, hear, and communicate with clients, staff, and external representatives.
  • Close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus are required.
  • Ability to work independently and as part of an interdisciplinary team.
  • Knowledge of communication styles, empowerment and self-advocacy techniques, participatory planning techniques, crisis intervention techniques, advocacy techniques, physical personal management skills, and household management skills.
  • Certified in cardiopulmonary resuscitation within two weeks of hire, First Aid within two weeks of hire, Medication Administration Program within 150 days of hire, Positive Behavior Supports Tier 1 within 150 days of hire, and Safety Care within 90 days of hire.
Responsibilities
  • Provide overnight program activities in a community living arrangement.
  • Provide individual and group support to persons served and teach skills that enhance independence and quality of life.
  • Ensure a safe, secure, and clean residential environment.
  • Assist in implementing treatment programs, including Individual Support Plans, Person-Centered Support Plans, and clinical programs, and participate in treatment planning.
  • Maintain records, collect behavioral and academic data, maintain progress notes, and document incidents, programs, and routine activities in a communication log.
  • Train and support persons served with activities of daily living, including showering, personal hygiene, dressing, and household chores.
  • Conduct fire and safety training and document it in the training log.
  • Monitor physical and emotional needs and report changes to the supervisor.
  • Store, administer, document, and dispose of medication in accordance with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Participate in mandated training and staff meetings and assist with orienting new staff.
  • Ensure compliance with internal and external regulatory requirements and corporate and divisional policies and procedures, including safe work practices, workers' compensation, universal precautions, and exposure control.
  • Perform hands-on program tasks, maintain site safety and maintenance, and complete housekeeping, snow shoveling, and garbage removal as needed.
  • Provide transportation as necessary.
  • Maintain the required staff-to-person-served ratio and provide last-minute coverage as required.
  • Provide physical intervention and First Aid during behavioral or medical emergencies and serve as a reporter to the Disabled Persons Protection Commission.
  • Remain awake, alert, and dressed throughout the entire shift.
  • Perform other related duties as required.
