  • At least two years of relevant work experience.
  • Ability to maintain effective interpersonal relationships.
  • Ability to communicate effectively in oral and written form.
  • Skill in collecting, organizing, and analyzing data.
  • Proficiency in computer software, including Microsoft Office.
Responsibilities
  • Prepare clinical charts for new rheumatology patients by reviewing referral documentation, outside medical records, laboratory results, imaging, prior specialty evaluations, medication history, and other relevant clinical information before the initial visit.
  • Identify missing records, laboratory results, imaging, prior treatment information, or other documentation needed for an effective rheumatology evaluation, and direct Patient Navigators to obtain outstanding information.
  • Review referral and clinical information to identify patients who may need additional clinical review, expedited attention, or coordination before the scheduled visit, and escalate concerns to the APRN or physician as appropriate.
  • Assess barriers to care and coordinate resources to support timely access, continuity, and completion of the treatment plan.
  • Provide high-level care coordination for complex rheumatology patients, particularly those requiring multidisciplinary or cross-specialty management.
  • Serve as a clinical liaison among patients, caregivers, rheumatology providers, primary care providers, external specialists, hospitals, facilities, and other departments involved in patient care.
  • Provide patient and caregiver education, reinforce provider-directed treatment plans, and help patients understand next steps, testing, referrals, and follow-up needs.
  • Coordinate continuity of care for patients requiring outside records, specialty services, multiple healthcare resources, or complex follow-up.
  • Lead the day-to-day workflow of two Patient Navigators, including prioritization and assignment of record retrieval, PCP and facility outreach, routine patient follow-up, and documentation needs.
  • Review unresolved or escalated Patient Navigator cases and determine whether issues require Nurse Navigator intervention, Registered Nurse clinical intervention, or APRN or physician review.
  • Collaborate with Registered Nurses to ensure clinical issues identified during navigation or chart preparation are appropriately routed and resolved without transferring routine back-end nursing work to the navigation function.
  • Collaborate with the APRN and physicians regarding complex clinical coordination needs and escalate clinical concerns requiring advanced-practice or physician decision-making.
  • Monitor navigation worklists and outstanding action items to promote closed-loop resolution before or after patient appointments, as appropriate.
  • Support quality-improvement activities related to new-patient readiness, access, continuity of care, patient experience, and navigation outcomes.
  • Serve as a clinical resource to Patient Navigators, CBORs, and administrative staff regarding appropriate routing of patient-care needs.
  • Maintain professional knowledge through continuing education and adhere to University and unit-level policies, procedures, documentation standards, and patient confidentiality requirements.
