Responsibilities
Contact youth and families by telephone to introduce the OhioRISE Care Management Entity, explain OhioRISE, and obtain verbal consent to participate in care coordination when appropriate.
Document all outreach attempts and communication in accordance with OhioRISE, Medicaid, and organizational documentation standards.
Support Care Coordinators and supervisors with the management of initial activities.
Review documentation for completeness and accuracy.
Complete special projects assigned by leadership.
