Requirements
  • A current Physical Therapist Assistant license in the state of employment is required.
  • At least one year of experience as a Physical Therapist Assistant in an acute setting is required.
  • Ability to make appropriate physical therapy judgments.
  • Ability to identify situations and handle them with an appropriate solution.
  • Ability to prioritize work and complete tasks in a timely manner.
  • A valid driver's license and auto insurance with the employee listed as a driver are required.
  • The employee must meet all physical position requirements and provide evidence of a current negative tuberculin skin test or receive TB testing at orientation, followed by annual tuberculin skin testing unless prior documentation is available.
  • The employee must be free of physical or medical conditions that would limit or restrict performance of the essential functions.
  • The position requires reading, speaking, and writing English; communication skills; computer or personal digital assistant usage; hand and finger dexterity; hearing and vision; walking; sitting; bending; and standing.
  • The position requires driving, lifting up to 50 pounds with or without assistance, stretching, reaching, climbing, and stooping.
Responsibilities
  • Provide skilled services in patients' homes according to the Physical Therapist's plan of care.
  • Observe, record, and report patient reactions, changes, and progress.
  • Perform medically prescribed physical therapy procedures and diagnostic tests.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, and water.
  • Assess functional problems and provide interventions in conjunction with the Physical Therapist to improve patient functional status.
  • Communicate with physicians, therapists, and coworkers.
  • Participate in case conferences to promote coordination of care.
  • Recommend prosthetic and orthotic devices, evaluate patients' devices, make minor adjustments, or recommend modifications.
  • Educate staff on specific physical therapy modalities when appropriate.
  • Educate patients and families on therapy techniques, safety measures, and supportive activities.
  • Instruct patients and family members on the proper use of wheelchairs, braces, walkers, crutches, canes, and other prosthetic or orthotic devices.
  • Foster independence in patients and their families to prevent dependence on the agency.
  • Complete clinical documentation in accordance with agency protocol and Medicare and federal guidelines.
  • Document all aspects of treatment, assessments, and patient education.
  • Complete chart audits at the agency location as requested.
  • Communicate schedule changes to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service trainings and mandatory agency meetings.
  • Participate in orientation and training for new employees.
  • Drive to patients' primary locations for scheduled visits.
  • Attend assigned visit locations daily.
  • Document subsequent, discharge, evaluation, recertification, resumption-of-care, and start-of-care visits within the specified timeframes.
  • Complete and submit required documentation within company requirements.
  • Follow the plan of care within the scope of practice for a licensed Physical Therapist Assistant under state requirements.
