Requirements
  • The candidate must be an Advanced Practice Registered Nurse Practitioner currently licensed in the state of service and must maintain necessary collaborative agreements.
  • The candidate must meet all applicable state-specific requirements.
  • The candidate must demonstrate commitment to the hospice philosophy of care.
  • The candidate must have verbal communication skills.
Responsibilities
  • Provide face-to-face encounters for hospice patients and, at the patient's request, serve as the attending physician.
  • Provide nursing assessment, planning, and care to maximize patient and family comfort and health in accordance with the Interdisciplinary Plan of Care.
  • Fulfill the role of attending physician for hospice plan of care and admission when applicable, without certifying the terminal illness.
  • Receive patient assignments for face-to-face encounters and operate under the direction of the Executive Director and Hospice Medical Director.
  • Initiate communication with the interdisciplinary team and other agencies as needed to coordinate care and resource use for patients and families.
  • Communicate and collaborate with the Hospice Medical Director.
  • Maintain regular communication with the assigned case manager and obtain data on physical, psychological, social, and spiritual factors affecting the patient.
  • Document continued hospice eligibility based on collected patient data.
  • Maintain an up-to-date schedule of patient visits to provide timely face-to-face encounters.
  • Submit required paperwork to the office in accordance with Medicare and Good Shepherd Hospice policies and procedures.
  • Participate in the interdisciplinary team process to evaluate continued eligibility for hospice services at admission, when applicable, and during recertification.
  • Participate in continuing education for agency staff and the medical community regarding pain and symptom management.
  • Perform other duties as assigned.
