Requirements
  • Graduation from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; alternatively, graduation with a major in any field combined with qualifying experience or developmental-disability coursework.
  • One year of full-time experience as a social worker, case manager, or care coordinator in a public or private social service agency; alternatively, one year of experience in education or treatment of people served with developmental disabilities or related conditions.
  • At least one course focused on developmental disabilities, verified by an official college or university transcript.
  • A valid driver's license and an acceptable driving record.
  • A vehicle covered by current insurance.
  • Successful completion of required local and/or state background checks.
Responsibilities
  • Assess the areas of the people served's lives where assistance is needed.
  • Offer and coordinate resources for the people served.
  • Monitor service delivery and work with providers to ensure individual needs are met.
  • Develop service plans.
  • Attend regular meetings with people served and their interdisciplinary teams.
  • Document client-related work in case notes to create billable units.
  • Travel between Hennepin and Scott counties for in-person meetings.

  • A Licensed Social Worker or higher form of Board of Social Work licensure.
