Responsibilities
- Maintain familiarity with diagnosis, psychiatric and somatic history, treatment plans, medications, and needs for individuals on the assigned caseload.
- Assist Persons Served in pursuing individual goals.
- Complete Person-Centered Planning Functional Self-Assessments thoroughly, timely, and professionally.
- Participate in developing an individual treatment plan for each person on the assigned caseload.
- Assess each person's stage of change on an ongoing basis and apply appropriate interventions.
- Provide transportation, money-management assistance, diet coaching, shopping, cleaning, assistance with activities of daily living, and life-skills instruction.
- Support individuals in recovery from mental illness, physical illness, and substance abuse using the Recovery Model.
- Ensure all work shifts are covered during the Program Manager's absence.
- Maintain 24-hour emergency phone coverage during the Program Manager's absence.
- Direct team activities, conduct scheduled team meetings, distribute meeting minutes, and provide staff support.
- Review timesheets for accurate hours, time off, overtime approval, and calculations.
- Oversee general staff functions and the condition of residences.
- Coordinate operational requests and report unsafe or unfit residential conditions.
- Coordinate Persons Served somatic and psychiatric needs with rehabilitation specialists, day-program counselors, the clinical nurse specialist, primary-care providers, the pharmacy, and the person served.
- Follow established medication-monitoring protocols and communicate pertinent issues to the nurse and Program Manager.
- Inspect residential medication supplies as agreed upon by the team.
- Assist with move-ins, including attending referral, discharge, and admission meetings; preparing residences; and coordinating receipt of move-in kits.
- Identify early warning signs of psychiatric or physical decompensation and take appropriate action.
- Diffuse and address behavioral or psychiatric crises to ensure safety.
- Distinguish between stressors and exacerbation of mental illness.
- Identify and promote the use of coping mechanisms.
- Perform other assigned duties, responsibilities, and special projects.
- Practice Recovery Model interventions in daily interactions and contribute positively to the work team.
