  • Graduate of an approved school of nursing.
  • Must hold and maintain a current Florida Registered Nurse license.
  • Handle with Care Certification is required for the Behavioral Health Unit within 90 days of hire; the training and education will be provided onsite.
  • Three years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
  • Demonstrated organizational skills, verbal and written communication skills, ability to lead and coordinate a diverse group in a fast-paced environment, critical thinking and problem-solving skills, and computer literacy.
  • Ability to manage multiple priorities and meet goals using problem-solving skills.
  • Ability to maintain confidentiality of staff information, data, and patient information.
  • Maintain reasonably regular and punctual attendance consistent with applicable policies and laws.
Responsibilities
  • Assess assigned patients initially and concurrently, including medical necessity screening and submission for Physician Advisor review.
  • Coordinate care by reviewing the admitting diagnosis, medical history, current treatments, age, payment source, resources, support systems, anticipated needs, expected length of stay, appropriate service level, special or personal needs, and other relevant information.
  • Assign an initial Diagnosis-Related Group to determine geometric mean length of stay, and monitor and manage length of stay and transition planning through assessment, reassessment, and application of InterQual guidelines.
  • Lead and facilitate multidisciplinary patient-care conferences and Complex Case Reviews.
  • Manage concurrent disputes and make appropriate referrals to other departments; identify and refer complex patients to Social Work Services.
  • Communicate the care plan with patients and families.
  • Identify and document potentially avoidable days, and identify and report overutilization and underutilization.
  • Ensure compliance with federal, state, local, and Joint Commission regulatory standards, including review requirements for managed contracts, Medicare, Medicaid, and campus-related admission and continued-stay approval.
  • Adhere to the Utilization Management Plan and integrate national care-management standards for utilization management, transition management, care coordination, and education of physicians, patients, families, and caregivers.
  • Communicate appropriately and promptly with the interdisciplinary team and third-party payers.
  • Prioritize work in assigned areas to focus on high-risk, high-cost, and problem-prone areas.
  • Develop collaborative relationships with patient business, nursing, physicians, and patients or families to facilitate movement through the continuum of care.
  • Monitor and evaluate data, fiscal outcomes, and other relevant information to develop and implement improvement strategies.
  • Forward identified quality or risk issues appropriately.
  • Maintain positive relationships with outside and onsite reviewers and payer representatives.
  • Identify cultural, socioeconomic, religious, and other factors that may affect treatment.
  • Involve the patient's family in treatment-plan development as appropriate, explaining procedures, therapies, systems treatment plans, and discharge plans in terms appropriate to the patient's and family's age, development, and education.
  • Review patient discharge plans at multidisciplinary meetings or staffing sessions to facilitate communication with other healthcare team members.
  • Participate in educational programs, review current literature, and attend workshops to support professional growth.
  • Maintain organized and retrievable work records and documentation while protecting confidential data and patient information.
  • Review current literature regularly, maintain and update reference materials, and stay current on relevant reimbursement information.
  • Serve on committees and task forces to promote quality, cost-effective care for the patient population.
  • Perform other duties as assigned or required.

Florida Cancer Specialists & Research Institute has opened a new 31,000-square-foot facility at Orlando Health Institute Square in St. Petersburg, expanding cancer care access in Pinellas County.
