About the job
- An active New York State Licensed Practical Nurse license is required.
- At least six months of home care experience is required.
- Current CPR/BLS certification is required.
- Strong clinical and communication skills are required.
- The candidate must be dependable, able to work independently, and able to lift up to 50 pounds as needed.
- Provide nursing care and administer prescribed medications and treatments.
- Perform G-tube/PEG care, tracheostomy care, and suctioning as needed.
- Monitor patients’ conditions and promptly report changes in health status.
- Provide ventilator support when required and appropriately trained.
- Educate patients and caregivers and maintain accurate clinical documentation.
