Requirements

Responsibilities
  • Ensure quality delivery of social worker services and assist in providing home health care services that reflect the Agency's philosophy and standards of home health care.
  • Assist in evaluating referrals for admission to the home health program, including identifying patients who would benefit from services, serving as a resource to hospital social service departments, nursing personnel, and discharge planners, and investigating patients' financial situations as appropriate.
  • Assist in identifying patients' needs and formulating an initial plan of care for patients admitted into the Agency's service by assessing patients, their families, support systems, and home situations; identifying needs or potential needs; and working with nurses or other health team members to formulate comprehensive plans of care.
  • Explore available community resources, assist patients in qualifying for eligible programs, help patients use available resources to obtain maximum program benefit, and follow up Adult Protective Services referrals with letters to Department of Social Services directors.
  • Counsel and support patients by helping them use or build support systems, assisting patients and families with the emotional aspects of illness and conditions, helping them accept medical conditions and comply with medical recommendations, and assisting with Advance Directives.
  • Participate in multidisciplinary team meetings, reevaluate goals and plans of care, offer suggestions to better meet patients' needs, assist health team members with emotional factors related to patients' health problems, and serve as a resource for health team members in planning patient care.
  • Maintain required medical records by documenting observations, actions taken, patient progress, and responses to care in a timely manner using paper or electronic means.
  • Keep physicians informed of patients' progress by forwarding appropriate documentation concerning progress and summaries of care at regular intervals and discussing patients with physicians when appropriate to clarify goals, objectives, and plans of care.
  • Perform other duties as assigned by management.
