Requirements
  • Registered Nurse
  • Licensed Practical Nurse with 2 years lived experience
  • Baccalaureate degree with 2 years lived experience
  • Associate degree with 4 years lived experience
  • Certified Medical Assistant with 4 years lived experience
  • High School Diploma REQUIRED
  • 1 year of relevant experience if candidate has bachelor's degree in related field
  • 2 years of relevant experience if candidate has associate's degree or is a Certified Peer Recovery Specialist
  • 4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant
  • 4 years of relevant experience, either clinical or lived, accepted in lieu of bachelors, associates or relevant certification
Responsibilities
  • Provide ongoing supportive services to patients with substance use disorders including education, service coordination, service linkage, and support within the healthcare, social service, and justice systems
  • Provide ongoing care-coordination and case management services
  • Guide linkage to appropriate clinical services including medication-assisted treatment, behavioral health treatment, and social services to individuals with Substance Use Disorder
  • Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder
  • Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment to reach and maintain optimal outcome
  • Provide continuity of care by meeting patients in various clinical care settings including emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments
  • Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc.
  • Perform outreach by meeting with patients in the community and act as allies and advocates for patient services
  • Attend Addiction Medicine clinical and programmatic conferences and meetings as required
Desired Qualifications
  • Lived experience navigating the criminal-legal system preferred
  • Bilingual candidates preferred
