Requirements
  • A Bachelor of Science in Nursing is required.
  • A current Ohio Registered Nurse license is required; an Indiana Registered Nurse license is a plus.
  • At least five years of MDS experience in a long-term care or nursing home environment is required.
  • Experience in post-acute environments is a plus.
  • A proven track record of strong MDS skills, including casemix management, PDPM management, and quality measures, is required.
  • Experience with electronic medical records is required; PointClickCare experience is highly preferred.
  • The ability to be flexible and adapt quickly to change is required.
  • Strong literacy, writing, documentation, communication, and interpersonal abilities are required.
  • The ability to cope with mental and emotional stress related to working with ill, disabled, elderly, or emotionally upset persons is required.
  • The ability to communicate in English is required.
Responsibilities
  • Complete MDS assessments and care plans effectively.
  • Discuss elder observations and concerns with the appropriate coordinator.
  • Generate and update the monthly MDS calendar and notification process for support staff.
  • Provide coaching and direction to staff as needed.
  • Conduct discharge planning for residents and elders, including arranging home services upon discharge.
  • Complete resident and elder certifications and recertifications.
  • Facilitate weekly Medicare and therapy utilization review meetings.
  • Act as the therapy liaison.
  • Provide training materials to staff and the team when appropriate.
  • Collaborate in developing new processes, forms, and systems.
  • Prepare for and actively participate in morning meetings.
  • Assist in managing the survey process when required.
  • Complete special projects assigned by the Vice President of Clinical Quality when an interim transition role is not needed.
  • Perform other duties as assigned.
Desired Qualifications
  • An Indiana Registered Nurse license is a plus.
  • Experience in post-acute environments is a plus.
  • PointClickCare experience is highly preferred.
