Requirements
  • Valid state Licensed Practical Nurse (LPN) license.
  • Valid driver's license and use of an automobile due to required travel.
  • Completion of annual MUI (Incident) training as required by Ohio Medicaid, the Ohio Home Care Program, and the Department of Developmental Disabilities.
  • Ability to read and understand agency documents, including Plans of Care, flow charts, client charts, policies, and procedures.
  • Ability to use a computer, laptop, or tablet.
  • Ability to think independently, recognize potential problems, gather information, and determine when to contact a supervisor.
  • Ability to provide exceptional client care and customer service while following agency and program rules.
Responsibilities
  • Administer medication in accordance with standards of practice.
  • Encourage social and rehabilitative skills, self-care, and independence.
  • Perform client treatments in accordance with the Plan of Care.
  • Assist consumers in learning appropriate self-care techniques.
  • Prepare clinical and progress notes documenting care provided.
  • Monitor clients, document appropriately, and report changes in client condition to the Clinical Director or Clinical Supervisor.
  • Promote activities that achieve a safe and healthy environment.
  • Use aseptic technique when performing client procedures and care.
  • Collaborate with clinical team members to develop and improve the Plan of Care.
  • Assist clients in achieving established Plan of Care goals developed by the registered nurse (RN).
  • Maintain productivity according to agency standards.
  • Maintain the required license, continuing education units (CEUs), and mandatory employment requirements.
  • Participate in client care meetings, in-services, orientation, and staff meetings.
  • Provide services in accordance with the agency's philosophy, standards, policies, and scope of practice.
  • Participate in performance improvement activities and adhere to Health Insurance Portability and Accountability Act (HIPAA) guidelines.
  • Perform other duties as assigned.
