Requirements
  • The candidate must hold a current license for the state location of employment.
  • The candidate must have 2 years of experience in an acute care or outpatient setting requiring comprehensive assessment skills.
  • The candidate must have experience coordinating care among physicians, clients, and referral sources.
Responsibilities
  • Observe and report deviations in client health status using age-appropriate norms and principles of growth and development.
  • Provide occupational therapy services to clients in their homes according to the plan of care established by the Occupational Therapist and physician, under the Occupational Therapist’s direction.
  • Provide occupational therapy to develop or restore function and maintain maximum performance in accordance with the plan of care.
  • Instruct clients, families, and other health care personnel in occupational therapy programs, including special techniques and adaptive devices.
  • Guide clients in therapeutic, creative, and self-care activities to improve functional abilities.
  • Use activities and exercises to improve upper-extremity range of motion, strength, and coordination as directed by the Occupational Therapist.
  • Provide sensory or perceptual motor training, activities of daily living training, and splinting or other special equipment to improve physical functioning or prevent deformities.
  • Identify unmet client needs and report them to the Occupational Therapist and the Vice President of Clinical Services or Director of Professional Services.
  • Record and report changes in client condition or reactions to treatment to the supervising Occupational Therapist, physician, and other health care personnel.
  • Participate in case conferences and discharge planning.
  • Use electronic documentation for client care and comply with agency policies regarding timely submission of documentation.
  • Perform other duties as assigned in response to client or agency needs.
Desired Qualifications
  • A minimum of 1 year of experience in a home health setting is preferred.
