Requirements
- The pharmacist must possess current licensure through the applicable state board of pharmacy where services will be provided, including West Virginia, Pennsylvania, Maryland, or Ohio licensure as applicable.
- The pharmacist must obtain Basic Life Support certification within 60 days of the hire date.
- The pharmacist must walk and stand for long periods of time.
- The pharmacist must be able to lift 30 pounds and push 50 pounds.
- The pharmacist must have dexterity to finger, handle, and reach.
- The pharmacist must have time management skills.
- The pharmacist must have verbal and non-verbal communication skills.
- The pharmacist must have teamwork skills.
Responsibilities
- Provide pharmaceutical care through direct patient care, operational, educational, and administrative activities.
- Monitor drug therapy by participating in medical rounds or evaluating patients’ medication regimens, medical problems, laboratory data, and special procedures.
- Provide leadership and patient care in assigned areas, guide providers and patients on care practices and guidelines, and assure safe and effective drug use through clinical involvement.
- Assist nursing and other disciplines or departments with admission and discharge planning and patient education concerning appropriate medication use.
- Provide drug information and educational programs to medical and nursing staff, other health care practitioners, and pharmacy staff.
- Maintain professional competence by attending and presenting at conferences, completing continuing education, reading current professional literature, and publishing clinical or practice-related experience or research.
- Participate in administrative, quality assurance, performance improvement, clinical research, and medication use evaluation activities.
- Review each order for accuracy and appropriateness.
- Evaluate compliance with medication safety procedures and suggest improvements based on best practices, including Institute for Safe Medication Practices guidance.
- Document medication errors, unsafe conditions, and near misses, and promote a constant focus on medication safety.
- Dispense medications from provider orders and ensure that drug distribution policies and procedures are followed.
- Be responsible for drug use control in assigned service areas and identify related problems while offering ideas for resolution.
- Make decisions related to the provision of pharmaceutical services in the assigned service area.
- Detect, report, and help minimize or manage adverse drug reactions and medication errors.
- Participate in management of acute and chronic disease states, medical emergencies, and adverse drug reactions.
- Communicate relevant findings and recommendations and document them in the clinical information system.
- Maintain records according to legal requirements and hospital policies and procedures.
- Supervise technicians in drug preparation and distribution duties.
- Provide objective performance information about co-workers to facilitate performance management.
- Participate in orientation and education of pharmacy students, pharmacy residents, and new department employees.
- Participate in educational and scholarly drug-therapy activities to enhance knowledge among pharmacy and other health professionals.
- Participate in planning and implementing pharmacy changes and improving productivity through staff meetings and projects.
- Complete and submit written assignments, continuing education documentation, and other assigned items in a timely manner.
- Successfully complete annual clinical competency evaluations.
- Complete annual education related to specialty accreditation requirements through applicable training and competency validation.
Desired Qualifications
- A Doctor of Pharmacy (Pharm.D.) from an Accreditation Council for Pharmacy Education-accredited school of pharmacy is preferred.
- Completion of a postgraduate year two residency in an applicable clinical field is preferred.
- Completion of a postgraduate year one residency and board certification in an applicable clinical field is preferred.
- Completion of a postgraduate year one residency and three years of relevant pharmacist experience is preferred.
- Three years of relevant pharmacist experience and board certification in an applicable clinical field is preferred.
