Requirements
  • Have at least 2-3 years of experience in a primary care or value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations.
  • Must have an active and unrestricted Florida Advanced Practice Registered Nurse (APRN) license.
  • Must have a Drug Enforcement Administration (DEA) license or be eligible to obtain it within two months of the date of hire.
  • Have a strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model.
  • Have excellent documentation and patient relationship-building skills.
  • Be proficient with electronic medical record (EMR) systems; experience with eClinicalWorks (eCW) is a plus.
Responsibilities
  • Provide personalized, preventative senior primary care.
  • Spend quality time with each patient while managing fewer patients per day.
  • Make autonomous clinical decisions.
  • Work Monday through Friday during 8:00 a.m. to 5:00 p.m. hours with no scheduled on-call duty.
  • Deliver care within value-based insurance contracts and workflows.
  • Support patient quality of life and satisfaction.

Palm Primary Care Expands Into Dallas County With Acquisition Of Two Primary Care Clinics

FORT WORTH, Texas--(BUSINESS WIRE)--Palm Primary Care Texas (“Palm”), part of the MBF Healthcare Partners II, L.P. (“MBF”) portfolio and Florida-based Palm Medical Centers, has acquired the assets of two primary care practices located in Dallas and Duncanville, from Texas Health Physicians Group. While the transaction has closed, the full transition of staff and clinics to the Palm name will occur over the next several months. With this acquisition, Palm has now opened or acquired six primary care clinics in Tarrant and Dallas counties since June 2023. The strategic expansion is a key part of having a meaningful impact on the health of Medicare beneficiaries throughout North Texas by 2025. The first clinic, located at 3920 W Wheatland Rd Ste 152 in south Dallas, is led by Bane Smith, MD and Eliazar Garcia Alvarez, MD, both of whom have served at the clinic for over 20 years
