About the job
- A Doctor of Medicine or Osteopathy is required.
- A current unrestricted state license to practice medicine is required.
- Board certification in family practice, internal medicine, geriatric care, or palliative care is required.
- Prior supervisory or management experience is required.
- Reliable transportation, a valid driver's license, and valid automobile insurance coverage are required.
- Demonstrated interpersonal, supervisory, and management skills, with the ability to communicate effectively, are required.
- Proven organizational development and project management skills are required.
- Working knowledge of home health care regulations and reimbursement, along with proven leadership abilities, is required.
- Conduct comprehensive medical evaluations, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.
- Coordinate care with patients, families, and interdisciplinary healthcare teams.
- Provide medical leadership and collaborative oversight for Nurse Practitioners and Physician Assistants to support high-quality, coordinated care.
- Mentor fellow clinicians.
- Provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings.
- Visit partner facilities in the service area once a week; physicians are assigned to two to three facilities.
- A minimum of two years of healthcare experience, combining acute care, geriatric, and/or long-term care practice, is preferred.
- Experience with geriatric patients is preferred.
