About the job

Requirements
  • Hold a college diploma (DEC) in social work techniques or a recognized equivalent.
  • Prior experience in home care or within the health network (CISSS/CIUSSS) is an asset, not a requirement.
  • Have a valid driver's license and be able to travel frequently between Forestville and Les Escoumins.
Responsibilities
  • Assess the psychosocial needs of users and their caregivers by visiting their living environments.
  • Participate in developing, implementing, and monitoring individualized intervention or service plans.
  • Provide psychosocial support, attentive listening, and assistance to users experiencing loss of autonomy or crisis situations.
  • Direct and refer beneficiaries to appropriate community, institutional, or material-support resources.
  • Collaborate closely with the interdisciplinary team, including nurses, occupational therapists, and physicians, to support users’ safe continued residence at home.
Desired Qualifications
  • Prior experience in home care or within the health network (CISSS/CIUSSS) is a major asset.
