Requirements
Graduate of an accredited nursing program.
A Colorado LPN license in good standing with the Department of Regulatory Affairs is required.
Basic Life Support certification is required.
A current driver's license and up-to-date car insurance are required.
Strong communication skills are required.
Strong computer skills are required.
Responsibilities
Provide one-on-one care to a single client in the client’s home and in the community.
Implement the client’s physician-ordered plan of care and nursing interventions in accordance with agency policies.
Perform assessments to monitor the client’s health status.
Notify the physician and Field Nurse Case Manager of changes in the client’s condition.
Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team.
Administer medications and treatments as ordered.
Document interventions and treatments performed.
Assist clients with activities of daily living, including bathing, toileting, dressing, and grooming.
Collaborate with the care team to implement and update the client’s plan of care.
Monitor client progress toward home health care goals.
Educate the client and family regarding the plan of care and nursing care needs.
Desired Qualifications
One to two years of home health care experience is preferred.
One year of nursing experience in a health care setting is preferred.

Essential Functions:

Minimum Education: Graduate of an Accredited Nursing Program required.
