Requirements
  • Obtain a Bachelor of Science in Nursing (BSN) or higher nursing degree within eight years of hire.
  • Have at least three years of experience in an acute care hospital setting.
  • Hold an active Registered Nurse license in good standing with the State of Michigan.
  • Complete Basic Life Support (BLS) certification by the end of core orientation.
  • Use word processing, spreadsheet, keyboard, presentation, and other job-relevant software.
  • Communicate effectively with healthcare providers and in an understandable, collaborative, and supportive manner with patients and families.
  • Demonstrate critical thinking, decision-making, problem-solving, prioritization, planning, organization, monitoring, and evaluation skills.
  • Work independently and remain self-motivated.
  • Use negotiation and conflict-resolution skills.
  • Perform sustained computer and detailed work, with regular walking, standing, stooping, bending, sitting, and occasional lifting of light objects.
Responsibilities
  • Move patients from admission through discharge without disrupting care by assessing, planning, implementing, coordinating, monitoring, and evaluating each assigned caseload.
  • Ensure care is appropriate to patient needs and hospital capabilities, and serve as an advanced clinical resource to patients, families, and staff.
  • Collaborate with the interdisciplinary team to provide a continuous, comprehensive, cost-effective course of care and monitor outcomes for continuous improvement.
  • Assess and identify patients at risk for post-hospitalization care needs; interview patients or families and review health records and psychosocial needs when indicated.
  • Develop, review, and revise patient transition plans with care coordination team members.
  • Monitor patient progress and document according to procedure; maintain ongoing contact with patients receiving care and round daily on assigned patients.
  • Identify readmissions and their causes, determine interventions to prevent further readmissions, and communicate plans to the multidisciplinary team.
  • Coordinate smooth transitions between services and document transitional plans that reflect patients' levels of care needs and choices.
  • Carry out transition plans, coordinate community post-discharge services, evaluate needs for hospital and extended-care resources, and make referrals when appropriate.
  • Liaise among patients, physicians, ancillary providers, and community services from diagnosis through post-discharge.
  • Develop, implement, coordinate, and communicate plans of care spanning acute care through transition out of acute care.
  • Maintain collegial relationships with physicians, nurses, and leaders; coordinate care using Pathways or Plans of Care and contribute to their ongoing development and revision.
  • Participate in assigned groups and committees.
  • Ensure appropriate use of community and outpatient resources to support post-discharge care.
  • Coordinate discharge plans and length of stay appropriate to care needs; facilitate quality, clinical, and financial outcomes; negotiate and procure needed services and resources; and intervene at key points in patients' care.
  • Evaluate case-management outcomes, including length of stay, readmissions, patient satisfaction, and financial variances; report pertinent variances and relate outcomes to healthcare reimbursement principles.
  • Track and trend outlier length-of-stay data to reduce outlier stays.
  • At Bronson Lakeview Hospital and Bronson South Haven Hospital, complete the annual Word day CBL on swing beds and understand the documentation differences and specific care required for swing-bed patients.
  • When assigned house-manager duties, coordinate and oversee hospital operations and provide clinical and administrative direction in the absence of unit leaders.
  • When assigned house-manager duties, resolve crises and conflicts, assess and intervene in problems requiring immediate attention, and inform department managers and directors of problems and resolutions.
  • When assigned house-manager duties, investigate complaints from patients, visitors, physicians, and staff and implement appropriate actions and follow-up.
  • When assigned house-manager duties, serve as the initial contact for atypical events, assess events, initiate appropriate actions to stabilize patients, and notify risk management, CSI, and department managers or directors.
  • When assigned house-manager duties, assess safety concerns and take appropriate action.
  • When assigned house-manager duties, coordinate admissions, transfers, and patient placement; monitor hospital capacity; and work with the Staffing Office on staff placement.
  • When assigned house-manager duties, conduct regular rounds of hospital departments and units and maintain utilization review.
Desired Qualifications
  • A master's degree is preferred.
  • Case Management Certification is preferred.
