Requirements
  • A High School diploma or GED is required.
  • One to two years of experience working in HIV/AIDS, behavioral health, substance use, or other chronic illnesses is required.
Responsibilities
  • Conduct outreach to providers, community members, and prospective clients to promote program services and identify clients.
  • Complete rental assistance applications, screen clients for eligibility, and conduct housing assessments and reassessments.
  • Coordinate case conferences with program staff to address client service needs and barriers to care.
  • Inspect properties and conduct home and field visits for clients receiving program services.
  • Interface with the Finance and Grants departments monthly, under the supervision of the Senior Program Supervisor, to monitor spending, budgets, program progress, and follow-up with funders.
  • Work with clients on budgeting and create or maintain a household budget within 30 days of enrollment.
  • Provide systems navigation and coordinate with medical providers, community-based organizations, and internal and external providers to address barriers to care and follow up on referrals.
  • Provide individual and group health education on treatment adherence and overcoming barriers to appropriate treatment engagement.
  • Complete clear and comprehensive documentation in accordance with agency and funder standards and requirements.
  • Screen referred clients, assess needs, and provide appropriate referrals.
  • Educate clients on navigating the Department of Social Services, community providers, and subsidy programs.
  • Schedule and escort clients to appointments when needed, make reminder calls, and motivate clients by sharing personal experiences.
  • Track service status to ensure proper delivery and follow-up.
  • Coordinate services with community agencies and provide information and referrals to clients and community service providers.
  • Negotiate and advocate for client services that support self-sufficiency and self-management.
  • Attend required department, team, all-staff, and other meetings.
  • Maintain confidentiality of patient, financial, and employee-relations information.
  • Perform other related duties as assigned.
