- A current New York State Licensed Practical Nurse (LPN) license is required.
- At least one year of recent hands-on experience is required.
- Basic Life Support certification is required.
- Case manage patients and assess, plan, develop, and implement comprehensive home health care plans.
- Measure vital signs, administer intravenous treatments, change dressings, and clean wounds as needed.
- Provide skilled nursing care, preventive and rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home settings.
- Collaborate with physicians, therapists, social services, pastoral care, home health aides, and other members of the patient care team for an assigned caseload.
- Provide patients and families with education on disease management and health promotion according to the physician’s plan of treatment.
