- Perform nursing assessments and prepare consumers for clinic visits.
- Interpret, process, and implement provider orders.
- Advocate for consumers and participate in ongoing evaluations of their progress toward treatment goals within the clinic setting.
- Maintain consumer records for the smooth, accurate, and safe implementation of medication orders, including ordering prescribed medications from the designated pharmacy for timely dispensing.
- Participate in multidisciplinary treatment team meetings as required.
- Provide general guidance and health education to individual consumers and family members.
- Complete clinical documentation in a timely manner consistent with agency standards.
- Use the electronic health record proficiently.
- Conduct assessments with consumers in the community, including at homes and day treatment programs, to provide continuity of care in the physical and psychological domains.
- Provide nursing coverage for other clinic locations and departments on an as-needed basis.
- Work collaboratively with community providers to provide integrated, whole-person care and address consumers' medical and treatment needs.
- Comply with applicable regulations, laws, rules, procedures, policies, standards, and grant requirements.
- Use de-escalation techniques to prevent or diffuse potentially disruptive situations.
- Obtain releases from primary care providers, specialists, other treatment facilities, emergency room visits, and hospitalizations as needed for consumer care.
- Complete prior authorizations and attestations for treatment medications.
- Perform rescue functions during emergencies, including administering basic emergency medical aid and evacuating people to safe locations.
- Establish and maintain effective working relationships with interdisciplinary teams.
- Demonstrate proficiency in laboratory procedures, injections, glucometer use, urine drug screening, and pregnancy testing.
- Follow up with consumers regarding laboratory results as outlined in company procedures.
- Establish rapport with consumers.
- Systematically gather data necessary for consumer treatment.
- Assist with determining an individual's readiness for treatment and change.
- Screen for substance toxicity, intoxication, and withdrawal symptoms as needed.
