- High School Diploma or GED.
- Certified Nursing Assistant in good standing with the State of Michigan, or completion of a State-approved training program within one month of hire and certification acquired within four months of hire as required by the state.
- Current CPR/BLS certification.
- All employees must be vaccinated or have laboratory-confirmed immunity for Measles, Mumps, Rubella, and Varicella.
- Employees must receive a flu vaccine during the flu season in the year they are hired and annually thereafter, or receive an approved medical or religious exemption.
- Provide supportive services to nurses and other staff as needed and perform assigned duties.
- Assist residents with, or perform, activities of daily living in accordance with care plans and established policies and procedures.
- Assist residents with lifting, turning, moving, positioning, and transportation into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
- Coordinate dining room services at assigned mealtimes, including setup and cleanup, meal tray delivery, feeding assistance, and documentation of meal intake.
- Deliver nutritional supplements to residents at assigned times and provide assistance as necessary to ensure intake; document intake accordingly.
- Assist with weighing residents according to facility policy and record weights in designated locations; report weight changes to the nurse and supervisor.
- Assist with tracking residents’ skin condition and report pressure areas, skin breakdown, or skin tears to the nurse and supervisor.
- Assist nursing staff in carrying out toileting program activities.
- Complete daily flow sheets indicating that specified tasks were completed and document each resident’s progress as directed by the supervisor.
- Keep the nurse and supervisor informed of factors that interfere with performing assigned work, including resident refusal or reassignment to a nursing unit.
- Report changes in a resident’s condition to the resident’s nurse and supervisor.
