Requirements
  • The successful candidate must be a graduate of an ACPE-accredited school of pharmacy.
  • A Doctor of Pharmacy degree is required.
  • Completion of a PGY1 Pharmacy Residency is required.
Responsibilities
  • Work closely with clinical and operational managers and pharmacists to implement quality patient care services.
  • Provide collaborative pharmaceutical care to patients in conjunction with physicians, nurses, and other health care professionals.
  • Provide direct patient care and conduct teaching and research activities to promote optimal, cost-effective patient care.
  • Maintain a liaison relationship with the NICU service through participation in patient care rounds and involvement in joint projects.
  • Identify problems and issues in specific patients and promote rational drug use on a case-by-case basis using evidence-based therapies, including therapy selection, monitoring and assessment of therapy outcomes, and sedation management.
  • Provide drug information to providers covering the service and its patients.
  • Report adverse drug reactions for patients on service.
  • Participate in multidisciplinary committees.
  • Provide patient education.
  • Develop and maintain Epic ordersets.
  • Develop, implement, and report results for continuous quality improvement and expense reduction programs related to the patients serviced, including medication use evaluations, treatment protocols and guidelines, and clinical pathways.
  • Act as secretary or member of multidisciplinary medical staff committees charged by the Formulary and Therapeutics Committee or another Medical Board Subcommittee with overseeing, developing, implementing, and managing policies for care delivered to the patients serviced.
  • Provide training and educational programs for PGY-1 and PGY-2 residents, undergraduate pharmacy students, and other health care providers.
  • Conduct research in areas of interest, especially those aligned with departmental goals and objectives, and submit manuscripts for publication or presentation at local, state, or national meetings.
  • Perform other duties as outlined by the Pharmacy Site Director, including departmental clinical on-call.
Desired Qualifications
  • Completion of a PGY2 Pharmacy Residency specialized in Pediatrics.
  • Three to five years of experience as a clinical pharmacist in NICU, PICU, or Pediatrics.
