Requirements
Graduation from an accredited nursing program is required.
A Colorado registered nurse license in good standing with the Department of Regulatory Affairs is required.
Basic Life Support certification is required.
Up-to-date car insurance and a driver's license are required.
Responsibilities
Provide one-on-one care to a single client in the client's home and while in the community.
Implement the client's physician-ordered plan of care and nursing interventions while adhering to agency policies.
Perform assessments to monitor the client's health status.
Notify the physician and Field Nurse Case Manager of changes in the client's condition.
Coordinate care with the Field Nurse Case Manager and other members of the client's multidisciplinary care team.
Administer medications and treatments as ordered.
Document performed interventions and treatments.
Assist clients with activities of daily living, including bathing, toileting, dressing, and grooming.
Collaborate with the care team on implementation and updates to the client's plan of care.
Monitor client progress toward home health care goals.
Educate the client and family regarding the plan of care and nursing care needs.
Desired Qualifications
One to two years of home health care experience is preferred.
One year of nursing experience in a health care setting is preferred.
Strong communication skills are preferred.
Strong computer skills are preferred.

Job Type: Full Time or Part Time
Salary: $30.00 - $38.00 per hour
