Requirements
  • The candidate must be at least 16 years old for Supported Community Living settings or at least 18 years old for Habilitation settings.
  • The candidate must hold a valid driver's license and be able to drive safely for transportation duties.
  • The candidate must have a high school diploma or equivalent.
  • The candidate must be able to work in the United States without sponsorship.
  • The candidate must remain awake during working hours.
  • The candidate must be able to assist with transfers and lift up to 35 pounds, with appropriate equipment or two-person assistance for heavier lifting.
  • The candidate must be able to walk and stand for up to eight hours, bend, stoop, and stretch frequently, and drive for up to eight hours when required.
  • The candidate must be able to work with difficult emotional and physical behaviors, including seizures and physical aggression.
  • The candidate must have sufficient distance and near vision, hearing, sound localization, and speech communication abilities.
Responsibilities
  • Collaborate with the HCBS Supervisor and Programmer to develop individualized goals and provide person-centered support that teaches skills, promotes independence, and encourages participation in recreational and community activities.
  • Use approved behavioral strategies to support individuals with challenging behaviors while respecting rights restrictions outlined by the interdisciplinary team and individual service plans.
  • Complete accurate, timely, and Iowa Code-compliant documentation in Setworks, recording and submitting required service details, interventions, and individual responses daily.
  • Provide hands-on support in home and community settings, including daily living assistance, transportation, mobility support, safety monitoring, property respect, and encouragement of choice and independence.
  • Administer and document medications according to the six rights.
  • Follow agency policies and procedures, maintain reliable attendance including weekends and holidays, complete required training, and communicate concerns or updates through appropriate channels.
