Requirements
  • An associate degree in Health Information Management or a related field, or equivalent experience in health information management, is required.
  • An HIT degree may be considered in lieu of experience.
  • Excellent personal computer knowledge and skills, including Windows and hospital information system software, are required.
  • Familiarity with medical terminology and medical record documentation requirements is required.
  • Data entry proficiency of 45 words per minute is required.
  • Proficiency with Microsoft Excel for managing spreadsheets is required.
  • Ability to perform production-oriented work under pressure with ongoing time constraints is required.
  • Ability to work unsupervised and remain self-motivated is required.
  • Working knowledge of Medical Staff Bylaws, Rules and Regulations, Centers for Medicare & Medicaid Services documentation standards, health system policies, and related patient medical record documentation requirements is required.
  • Knowledge of state and federal patient privacy laws, including the Health Insurance Portability and Accountability Act, is required.
  • Visual and auditory acuity is required for processing and following up on indexing issues.
  • Good hand coordination and manual dexterity are required.
  • Excellent analytical, organizational, interpersonal, problem-solving, written communication, and verbal communication skills are required.
  • An excellent attendance record, including punctuality, is required.
  • Ability to perform occasional reaching, pulling, pushing, bending, stooping, and lifting of approximately 5 to 10 pounds is required.
Responsibilities
  • Work effectively with medical staff, health care providers, and other personnel to expedite the ongoing and timely completion of patient-record documentation for regulatory, accreditation, and related standards, policies, and practices.
  • Review documentation required in patient charts, follow up with providers, and process transactions from designated work queues.
  • Follow up effectively and promptly with providers regarding priority chart-completion requirements, including pre-suspension and delinquent charts.
  • Work independently to identify methods for timely documentation review and obtain information or prompt completion of required patient-chart documentation.
  • Assist with health information management quality-review functions, chart analysis, fax-in queue transaction processing, and other priority functions as directed.
  • File and retrieve scanned charts and stock the Physician Lounge.
Desired Qualifications
  • Two years of experience in a health information management setting, including chart documentation analysis, is preferred.
