About the job
- A bachelor's degree in social work or a bachelor's degree in a human services field.
- One year of supervised social work experience in a health care setting working directly with individuals.
- Develop preliminary and comprehensive assessments, written plans of care, discharge plans, and progress notes, and review and revise these plans as necessary.
- Provide residents, family members, and resident representatives with information concerning resident rights and advance directives.
- Develop effective working relationships with residents, families, team members, physicians, community members, and others to collaborate, advocate, and problem-solve on behalf of residents.
- Ensure residents have access to prescriptions, durable medical equipment, and other services identified upon discharge.
- Refer residents to education regarding healthcare delivery and reimbursement systems, prescription drug programs, health and wellness programs, community agencies, public and private organizations, housing options, and other appropriate services.
- Participate in committees and performance improvement projects.
- Maintain regular and punctual attendance consistent with applicable organizational policies and federal, state, and local standards.
- Maintain compliance with organizational policies and procedures.
- A master's degree in social work is highly preferred.
- Licensed Master Social Worker credential is highly preferred.
