Responsibilities
  • Provides care management support to panel of patients who require specialty services integrated and in support of their overall plan of care
  • Interacts with and supports the specialty care providers, primary care providers and the interdisciplinary care team across the continuum of care
  • Assesses plans, implements, documents, coordinates, monitors, evaluates, and updates the plan of care by collaborating with all members of the health care team to provide evidenced based care, leveraging nationally recognized guidelines as appropriate
  • Researches, evaluates and recommends resources to meet medical and non-medical needs of patients and families
  • Works to establish collaborative processes that promote quality and cost effective care that optimizes the physical and psychosocial health of patients across the health care continuum
  • Responsible for supporting decisions that impact health care outcomes, resource allocation and customer experience
  • Acts as a primary contact for the care team to assist in navigation and complexity management
  • Engages in quality improvement initiatives and program development
  • Could require work and travel to multiple locations in support of the patient and department
  • Gathers all relevant data and obtains information by communicating with the patient, family, healthcare provider, other members of the healthcare delivery team and patient’s community support network including external healthcare providers and agencies

  • Initiates and implements plan modifications as necessary through monitoring and re-evaluation to accommodate changes
  • Incorporates evidence-based nursing practice and takes into consideration current statutes, rules and regulations when developing the plan of care
  • Supports planning across the continuum of specialty care services and in collaboration with other care management professionals in primary care and inpatient areas as patient needs those services
  • Supports planning with community resources and external healthcare agencies to provide broadest available integrated network of support as needs indicate
  • Maintains high level oversight of the specialty plan of care in conjunction with the overall integrated plan and assures goals are met and/or addressed
  • Works with patient, family, healthcare providers and community supports to coordinate needed services
  • Identifies barriers and works with patient and family to resolve them
  • Facilitates communication between patient, family and all members of the health care team
  • Assures health care benefits have been reviewed and plan has been coordinated with the insurance provider
  • Addresses complex communication and planning issues as patient receives services across the specialty care continuum
  • Leads and supports transition and discharge planning for patients moving between levels of care
  • Transition patient back to primary care provider and care manager when specialty services are no longer needed
  • Provide education and information for patient related to diagnosis, treatment, and quality of life
  • Follows the patient over time, across sites of care as long as specialty care is needed, to measure effectiveness of the plan
  • Adapts plan to meet changing needs
  • Communicates with patient, family and healthcare providers about changes in plan
  • Coordinates, implements and documents plan of care and outcomes
  • Documents plan of care in electronic medical record
  • Supervises and delegates to care management support staff tasks that contribute to the plan
  • Addresses barriers to the plan
  • Acts in an oversight capacity through all episodes of care, and collaborates with care managers and other healthcare professionals in other specialty or inpatient areas and the community to assure overall plan is met
  • Participates in staff, departmental meetings and assists with identification and resolution of problems, ideas and opportunities
  • Communicates issues and decisions relating to committee and project work to other team members, management, and sponsors
  • Participates in quality and performance improvement activities related to specialty care practice

Desired Qualifications
  • 2 to 5 years of nursing experience required with a minimum of 3 years of experience in area of clinical specialty, care coordination or care management
  • 0 to 2 years of clinical experience in outpatient and hospital settings
  • 0 to 2 years of progressive leadership through engagement in performance improvement and program development
  • Case Manager Certification or Specialty Certification preferred

Water pipe bursts at Abbott Northwestern Hospital, some appointments rescheduled. Updated: 25 minutes ago Published: April 9, 2026 - 7:44 AM Water main break at Abbott Northwestern. A water main break disrupted services Thursday morning at Abbott Northwestern Hospital and forced the rescheduling of some appointments. An Allina Health spokesperson confirmed the issue, saying a pipe ruptured at the Minneapolis hospital just before 1 a.m. Hospital facilities staff members quickly worked to fix the issue and limit the impact on patients, the spokesperson added, although water had to be shut off for a few hours. It has since been restored. The issue did cause some appointments to be rescheduled but affected fewer than a dozen elective procedures, according to Allina. All of those impacted patients have already been contacted, and anyone else who hasn't received a message should proceed with their appointments as scheduled. "The safety of our patients and care team members remains our top priority as we assess damage and address clean-up needs," the spokesperson said. "We are extremely grateful for the quick action of our care team members in responding to this incident." Allina buys Minneapolis medical office for $14.5M. Allina Health has purchased a medical office next to a south Minneapolis hospital, according to a recently released certificate of real estate value. The 76,499-square-foot property was sold by HCP Medical Office Buildings II LLC to Allina for $14.5 million. A business filings search shows that the seller is related to Healthpeak Properties, a Denver-based investor group that buys health care facilities. The facility has 7 floors plus a lower level, according to Brian Bruggeman, a senior vice president at Colliers, which brokered the deal. The building is located next to the Abbott Northwestern and Children's Hospital and is addressed at 920 E. 28th St. in Minneapolis, the CRV shows. Allina was already the tenant in the building, dubbed the Minneapolis Heart Institute Building. The nonprofit health system also housed administrative services, Allina Health Surgical Specialists and Allina Health Lung and Sleep in the building, according to its website. Reached for comment for this story, the Allina confirmed its purchase of the facility. "Given its location on the Abbott Northwestern Hospital campus, Allina Health purchased the building to secure the long-term positioning on the campus with no plans to change how it is used," Allina said in a statement. The purchase by the Minneapolis-based health system comes not long after a California nonprofit, Sutter Health, agreed to acquire Allina. As a part of this acquisition, Allina is set to become the "Upper Midwest Division" of Sutter. It will maintain the Allina name, and CEO Lisa Shannon will continue in her leadership position. The deal is set to close - pending regulatory approval - by the end of 2026, according to a press release from Allina. According to Healthpeak's website, the only other facility the group owns in Minnesota is the MHealth Fairview Clinic & Specialty Center in Maplewood. Finance & Commerce reached out to the seller for this story but did not hear back before time of publication. According to the CRV, the sale was a cash deal. The Catylist listing designates the 1986-built building as a Class B property. With its 76,499-square-foot layout, the price-per-square-foot works out to around $189.54. This price point is significantly below the top-of-the-market medical office properties in the west metro, such as the Arbor Lakes Medical Building in Maple Grove. Correction: An earlier version of this story contained an incorrect size for the building due to an inaccurate property listing. The Minneapolis Heart Institute Building has 76,499 square feet of space across seven stories and a lower level.

Allina Health to be acquired by California-based Sutter Health. Kyle Brown KSTP 1 hour ago (KSTP) - Allina Health, one of the largest health care providers in Minnesota, is being bought out by California-based Sutter Health, the health systems announced on Tuesday. While Allina would keep its name, board of directors and regional headquarters in Minneapolis, the nonprofit would fall under the purview of Sutter President and CEO Warner Thomas. The sale comes as 600 Allina doctors, physician assistants and nurse practitioners represented by Doctors Council - SEIU continue to negotiate their first contract since forming a union in October 2023. Union members have authorized multiple strikes, including a vote last week to approve an open-ended unfair labor practices strike, over ongoing frustrations at the bargaining table. Last year, Allina closed four clinics around the metro and shuttered its birth center in Faribault, consolidating its obstetric care services in southern Minnesota to the Owatonna Hospital. In their announcement, Sutter's leadership pledged to invest $2 billion in Allina to focus on improving outpatient care access, adopting AI tools and other technology to streamline processes and hiring more health care professionals. "We are incredibly excited for the opportunity to harness the collective strength of our two mission-driven organizations to make a difference in the lives of our patients, communities and care teams," said Lisa Shannon, Allina's president and CEO. "As one nationally leading, locally committed nonprofit health system, we will be uniquely positioned to be at the forefront of innovation, building upon the expertise of our physicians, advanced practice providers, nurses and team members to chart a new path for healthcare." The two organizations have signed a letter of intent, and they are expected to close on the deal by the end of 2026, barring any obstacles as Sutter conducts its due diligence.
