  • A bachelor's degree in a health-related field, or a bachelor's degree in a business field with two additional years of healthcare experience, or an associate degree with two years of experience in a variety of clinical settings, or an Oklahoma Licensed Practical Nurse license with three years of nursing experience, or six years of directly related experience in lieu of a degree.
  • Current Oklahoma Licensed Practical Nurse licensure or a current multistate license from a Nurse Licensure Compact member state, when using the LPN qualification path; alternatively, the specified bachelor's or associate degree pathways may be used in lieu of the LPN license.
  • Basic Life Support certification issued by the American Red Cross or American Heart Association within 30 days of hire.
Responsibilities
  • Identify potential candidates who could benefit from homebound services.
  • Educate patients and families before discharge about available home health care, skilled nursing, nursing home, rehabilitation, hospice, and home infusion services.
  • Facilitate communication about qualifying criteria for admission to post-acute hospital care venues.
  • Document referrals in the medical record according to departmental policy.
  • Serve as a resource to physicians, case managers, social workers, and the interdisciplinary team regarding post-acute care needs.
  • Educate patients about quality standards for patient satisfaction measures.
  • Maintain communication among patients, families, the RN Case Manager, and the Social Work Team regarding pending discharge arrangements.
  • Communicate appropriate medical records to receiving agencies or facilities and the appropriate payer.
  • Lead age- and developmentally appropriate patient education groups addressing the emotional, physical, and environmental needs of Behavioral Health and Addiction Recovery patients.
  • Participate in writing treatment plans with program staff and physicians.
  • Assist with utilization review by contacting patients' insurance companies and providing clinical information to authorize treatment.
  • Serve as a patient advocate for patients and involved individuals.
  • Complete or facilitate Emergency Detention paperwork and ensure its distribution and receipt by legally designated individuals.
  • Process orders, refills, or laboratory requests for the ordering physician.
  • Monitor patients' medications and provide education about their use and side effects.
  • Monitor vital signs as indicated by physician orders or when medically necessary.
  • Provide referral options for patients who are assessed but do not enter the program.
  • Complete comprehensive initial mental health assessments of patients' clinical, psychological, and financial needs using available resources.
  • Attend and participate in treatment team meetings.
