Requirements
  • Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work.
  • Comfort working independently in a fast-paced, community-facing environment.
  • Ability to manage outreach activity, relationships, and follow-through across multiple partners.
  • Strong organizational skills and comfort using mobile tools and customer relationship management systems.
  • A valid California Driver's License and active auto insurance meeting California requirements are required.
  • A reliable personal vehicle for daily work use is required.
  • Successful completion of a background check, including a motor vehicle records check, is required.
  • Must be able to travel up to 60–70% within the county to conduct in-person visits.
  • Must successfully complete a Testlify skills assessment.
  • Must have a reliable working laptop for the first 21 days of employment until the company-issued laptop is received.
  • Must have effective time-management skills.
  • Must have internet speed of at least 300 megabits per second download and 25 megabits per second upload.
  • Must be proficient in technology, including documentation systems, case management platforms, and communication tools.
Responsibilities
  • Identify and prioritize community-based organizations across more than 100 categories within assigned local markets.
  • Build and maintain referral-generating partnerships supporting access to Enhanced Care Management, Community Supports, Behavioral Health, Street Medicine, and Case Health Worker services.
  • Develop and sustain partner relationships within an assigned outreach territory to produce ongoing referrals.
  • Spend the majority of the workweek in the field meeting potential partners and engaging the community through in-person outreach.
  • Identify individuals who may benefit from Pacific Health Group services and facilitate warm referrals or enrollments through direct engagement.
  • Balance relationship building with consistent follow-through to support steady referral activity.
  • Accurately document outreach activity, referrals, and partnerships in customer relationship management tools.
  • Coordinate with internal care teams to ensure referrals are tracked and closed-loop.
  • Share observations and insights from the field to inform outreach strategy and improvement.

Pacific Health Group delivers integrated Medi-Cal care coordination in California, working with 39 health plans to serve over 38,000 members in 44 counties. Its flagship Enhanced Care Management coordinates medical, behavioral, and social services for people with the most complex health needs, supplemented by Community Supports like housing navigation, rent assistance, medically tailored meals, and home modifications. Street Medicine reaches unsheltered individuals, while Community Health Workers provide culturally responsive care navigation and advocacy; additional services include behavioral health support, medical transportation, and a Clubhouse for adults with mental illness.
