In this hybrid role, you’ll provide health support, wellness education, and staff training under the direction of a Registered Nurse (RN). You’ll build relationships through in-home visits, phone consultations, and collaboration with team members while helping staff feel confident supporting the health needs of the people we serve.
What You’ll Do

What You’ll Bring
An active nursing license: Current Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing.
Relevant experience: Experience in home or community-based care is preferred.
Strong communication skills: Ability to explain health information and procedures clearly to staff and team members.

Compensation & Schedule
Pay: $25.36–$28.80 per hour
Schedule: Full-time, primarily daytime hours
Work setting: Hybrid, with in-home visits, staff training, phone consultations, and team collaboration
