- A Bachelor's degree in Social Work or a related human service degree with two years of experience working with the people served and/or providing case management.
- Two years of experience working with individuals with mental illness, substance use issues, and/or a history of homelessness, or two years of experience working in the human services field.
- Working knowledge of computer programs including email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint, and the Windows operating system.
- Assist agency consumers with accessing needed community services through the development of a referral network, referrals, and follow-up.
- Provide case management to an assigned caseload.
- Develop and monitor individual service plans in conjunction with agency consumers.
- Assist agency consumers with problem-solving and crisis intervention.
- Accompany agency consumers on medical, psychiatric, and public benefit appointments as necessary.
- Act as an advocate for consumers as appropriate and encourage and support self-advocacy.
- Identify problems in daily living skills and provide hands-on assistance and/or referrals to address them.
- Support Facilities Management staff in ensuring maintenance of apartment standards outlined by S:US, OASAS, and other funders.
- Provide supportive counseling to agency consumers as needed.
- Conduct regular home visits and hospital visits as necessary.
- Maintain a strong working relationship with therapeutic support services to ensure quality of care.
- Document all services in compliance with agency standards.
- Assist with moves with consumers and staff, including lifting, packing, and cleaning.
- Work as a member of the social service team to develop off-site recreation, socialization, educational, and skill-building activities supporting community inclusion and integration.
- Serve as a liaison to community-based services and groups, including civic, social service, government, and business organizations.
- Participate in individual and group supervision, team meetings, case conferences, division meetings, and trainings.
- Adhere to the on-call cell phone system to facilitate crisis intervention, emergencies, and other situations requiring immediate action.
- Experience in group work, tenant organization, case management, referral, and/or community outreach.
- Experience or training in peer counseling or advocacy.
- Bilingual English and Spanish speaking ability.
S:US helps New Yorkers who face homelessness, disability, behavioral health and addiction challenges, poverty, criminal justice involvement, and trauma access a wide range of supports across New York City and Long Island. It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
