Responsibilities
  • Follow the home-care clientele, including palliative and end-of-life care for users in RI-RPA and those receiving intravenous antibiotics at home.
  • Plan, provide, and follow up on nursing care.
  • Collaborate with the interdisciplinary team to ensure continuity of care.
  • Provide education to users and their families.
  • Maintain complete and compliant clinical documentation.
