Requirements
  • Licensed Registered Nurse (RN) or Licensed Nurse Practitioner
  • 3-5 years of experience in a healthcare, patient oriented customer service position
  • Experience with the populations similar to the Medicare Enrollee population, including community based and facility-based people with medical, LTSS and/or behavioral needs
Responsibilities
  • Supervising the Care Coordinator Registered Nurse positions, Care Coordinator Social Work positions, and the Navigator positions
  • Collaborating with team members and with providers to deliver medical, behavioral, psycho/social care, coordinating long-term services and supports, home and community-based services, health and wellness classes, and other related services and supports to members
  • Advocating for members' rights and serving as a resource to the Director of Care Management, members, their families, and support systems
  • Travel is required; travel percentage is based on patient and enrollment needs
Desired Qualifications
  • Case management experience preferred
  • Disease management experience preferred
