Requirements
  • Nurse Practitioners must have a Master of Science in Nursing and a current Illinois Advanced Practice Nurse license with full practice authority.
  • Physician Assistants must have a Master of Physician Assistant Studies or equivalent and a current Illinois Physician Assistant license.
  • Current Basic Life Support and Advanced Cardiovascular Life Support certification is required.
  • A current driver's license and reliable transportation are required.
  • A minimum of 5 years of clinical experience is required.
  • Knowledge of harm reduction principles and comfort working in non-traditional, street-based settings are required.
  • Excellent interpersonal skills and an empathetic approach aligned with the organization's mission are required.
Responsibilities
  • Conduct physical assessments and provide appropriate treatment for acute and chronic conditions during street outreach and mobile van shifts.
  • Administer vaccinations, dispense medications from the Chi-Care Med formulary, and provide prescriptions within the scope of services.
  • Perform infectious disease point-of-care screening and wound care as needed.
  • Ensure accurate, timely documentation in the electronic health record and maintain compliance with confidentiality standards.
  • Identify patients' social needs and collaborate with social workers to connect patients with partner organizations and higher-level care.
  • Participate in team meetings, trainings, and quality improvement efforts.
  • Support students and volunteers as needed.
Desired Qualifications
  • Community health, emergency, or primary care experience is preferred.
  • Experience with people experiencing homelessness, substance use, or mental health conditions is strongly preferred.
  • Spanish language skills are preferred but not required.
