Requirements
  • Graduate of an Accredited School of Nursing Required
  • Unencumbered LVN License in state practicing
  • Reliable means of transportation
  • Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations
Responsibilities
  • Provide and document in-person supervision and instruction to home care assistants and home health aides
  • Instruct the participant, family and caregivers regarding the disease process, self-care techniques, and prevention strategies
  • Conduct and Perform home safety assessments
  • Deliver and document nursing interventions
  • Support to the clinic staff and home health staff work to manage smooth care transitions between settings
  • Must be willing and could work a varied schedule that may include evening nights, weekends and overtime
