Requirements
  • The candidate must be a licensed nurse in good standing in the State of Ohio.
  • The candidate must hold a current Basic Life Support cardiopulmonary resuscitation certification.
  • One year or more of experience as a registered nurse or licensed practical nurse in home health is preferred.
  • The candidate must be able to read and understand plans of care, flow charts, patient charts, agency policies and procedures, and other agency documents.
  • The candidate must have knowledge of medical terminology.
  • The candidate must be able to use a computer, laptop, or tablet.
  • The candidate must have mathematics skills sufficient to add, subtract, multiply, and divide whole numbers.
  • The candidate must maintain patient confidentiality and protect patient privacy.
  • The candidate must be able to think independently, recognize potential problems, gather information, and determine when to contact a supervisor.
  • The candidate must provide exceptional patient care and customer service within agency and program rules.
  • The candidate must have typing and computer skills.
Responsibilities
  • Administer medications in accordance with standards of practice.
  • Encourage social and rehabilitative skills, self-care, and independence.
  • Perform patient treatments in accordance with the plan of care.
  • Assist patients in learning appropriate self-care techniques.
  • Prepare clinical and progress notes documenting care provided.
  • Monitor patients, document appropriately, and report changes in patient condition to the Clinical Director.
  • Promote activities that achieve a safe and healthy environment.
  • Use aseptic technique when performing patient procedures and care.
  • Practice Standard Precautions at all times.
  • Work with other clinical team members to develop and improve the plan of care.
  • Assist patients in achieving established goals in the plan of care developed by the registered nurse case manager.
  • Maintain productivity according to agency standards.
  • Participate in patient care meetings, in-service training, orientation, and staff meetings.
  • Request supervisory assistance when appropriate.
  • Adhere to agency policies and procedures.
  • Provide services in accordance with the agency's philosophy, standards, policies, and scope of practice.
  • Participate in Performance Improvement activities and follow HIPAA guidelines.
