Requirements
  • Registered Nurse qualification.
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • Current license to practice nursing in the state of employment.
  • Current cardiopulmonary resuscitation certification.
  • Current automobile insurance and a valid driver's license.
  • Knowledge of terminally ill patients and their families and an understanding of hospice care.
  • Ability to apply knowledge of the special needs of hospice patients and families.
  • Knowledge of the roles of disciplines providing hospice services.
  • Patient assessment skills, knowledge of community resources, oral and written communication, documentation, and program management.
  • Knowledge of general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC.
  • Ability to operate nursing assessment tools, computers, facsimile equipment, copiers, and cell phones or beepers.
  • Must maintain transportation for work.
  • Ability to support patients and families during life-and-death issues and times of crisis.
  • Ability to communicate effectively with people from all socioeconomic backgrounds.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients' homes, assisted living facilities, or other settings.
  • Perform comprehensive initial and ongoing patient assessments and reassessments, including in-person and telephonic registered nurse assessments and screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including patient visit frequency and risk identification.
  • Coordinate care across interdisciplinary teams, including physicians and nurse practitioners.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and loved ones.
  • Teach caregivers about safe care, disease processes, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
  • Communicate changes in patient condition and signs or symptoms of decline to appropriate providers.
  • Support home health, palliative, and hospice referrals and transitions in levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, and policy standards.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the care plan requires team participation.
  • Participate in on-call and patient-support activities and conduct on-call services in a clinically competent and responsive manner.
  • Support quality-improvement and scheduling initiatives.
