Requirements
  • At least 3 years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a related public-sector human services or behavioral health care field providing community-based services to children, youth, and their families or caregivers.
  • Background and experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Clinical and cultural competence, with the training and experience necessary to manage complex community cases across child-serving systems.
  • Qualifications and experience working with children and families experiencing serious emotional disturbance, trauma, or co-occurring behavioral health disorders and engaged with one or more child-serving systems.
  • Ability to travel within the assigned region, to multiple offices, and to family locations using a personal vehicle, with a valid driver's license and proof of insurance.
  • Knowledge of human resources, personnel practices, and human relations.
  • Ability to work effectively with individuals from diverse backgrounds and experiences.
  • Ability to maintain confidentiality and provide objective, unbiased, trustworthy, and accessible service.
Responsibilities
  • Use clinical, operational, and technical expertise to manage and provide Moderate Care Coordinator care coordination.
  • Manage 1915(c) home- and community-based waiver service coordination in accordance with federal requirements.
  • Collaborate with the Child and Family Team and share responsibility for developing, implementing, monitoring, and evaluating a child- and family-centered plan.
  • Develop and leverage partnerships with non-health child-serving systems.
  • Contact the child's family within one business day of referral to Moderate Care Coordination.
  • Complete and submit initial, changed, and transition child- and family-centered care plans to the OhioRISE plan for review and approval according to standards.
  • Respond to member needs as appropriate.
  • Conduct an initial comprehensive home-based assessment and develop the child- and family-centered care plan.
  • Complete a Child and Adolescent Needs and Strengths assessment every 180 calendar days or after a significant change in behavioral health needs or circumstances.
  • Convene and facilitate the child and family team.
  • Develop a crisis safety plan.
  • Review the child- and family-centered care plan every 30 days or after a significant change in member needs or circumstances.
  • Monitor the child- and family-centered care plan.
  • Perform referrals and linkages to appropriate services across the continuum of care.
  • Maintain a caseload ratio of no more than 25 OhioRISE members receiving Moderate Care Coordination services.
  • Complete the high-fidelity wraparound training program and state-required training program provided by the Center of Excellence.
