- Registered Nurse degree.
- Current College of Nurses of Ontario registration in good standing.
- Graduation from the Nurses Specialized in Wound, Ostomy & Continence Canada program; the Canadian Nurses Association exam may be pending.
- Several years of escalating experience managing wounds and ostomies.
- A valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- Current CPR certification.
- A current immunization record including a two-step tuberculosis test.
- A current vulnerable sector check and a clear background check are required.
- Provide wound and ostomy consultation for patients in the clinic and community.
- Use assessment, prevention, and intervention knowledge regarding wound infections and ostomy care.
- Care for patients with consuming wounds, ostomies, and continence needs, including lower-leg assessment with ankle-brachial pressure indices.
- Critically appraise wound-care literature and apply research evidence to practice.
- Provide classifications and indications and use appropriate utilization to reduce visit frequency and obtain desired outcomes.
- Practice conservative sharp wound debridement and use other debridement methods.
- Provide edema management, including bandaging based on complete lower-leg assessment.
- Monitor criteria for adjunctive therapy for complex or recalcitrant wounds.
- Support patient safety by practicing according to policies and standards.
- Perform a thorough holistic assessment of each client and identify client needs using information from the client, caregivers, the home environment, the Ontario Health atHome referral, and other sources.
- Function as a member of a nursing team accountable for the nursing care it provides.
- Develop appropriate time-specific, measurable goals and outcomes in collaboration with clients or caregivers and in keeping with the Ontario Health atHome treatment goal.
- Lead the interdisciplinary team to achieve optimal patient outcomes.
- IIWCC or WCS certification may be considered and may correspond to a different pay-rate range.
- Exceptional interpersonal skills.
