Requirements
  • A bachelor's degree in a human service field, such as Child Psychology, Social Work, or Human Services, with at least one year of experience or training in social services, or five years of relevant Human Services experience.
  • Experience working with children and families with significant behavioral health needs.
  • A valid driver's license and state-mandated levels of auto liability insurance.
  • Eligibility to obtain CPR certification within 30 days of hire.
  • Eligibility to earn Wraparound certification as soon as possible, depending on statewide training availability.
  • Ability to continuously access an internet connection with 100 Mbps download and 20 Mbps upload speeds.
  • Ability to travel 65–70% within UCFS Healthcare’s service area.
  • Proficiency with Microsoft Suite, especially Outlook.
  • Ability to provide strength-based, client-centered services.
  • Ability to work in a fast-paced environment and handle multiple tasks simultaneously.
  • Strong communication and teamwork skills.
  • Robust interpersonal expertise across digital, in-person, and verbal correspondence.
Responsibilities
  • Provide outreach and engagement to clients referred to the program by conducting an initial phone intake with the family, collecting basic information, and presenting information to the team to determine eligibility.
  • Complete family assessments of strengths and areas of need through home visits and Ohio scales, and develop a care plan.
  • Provide weekly face-to-face direct services, including support, advocacy, referrals, and assistance with positive parenting, safety planning, basic life skills, and parenting a child with special needs.
  • Develop a list of interested parties identified by the family to participate in Child Specific Team meetings, lead those meetings multiple times during family involvement, and follow through on meeting recommendations.
  • Build and maintain community relations through education, advocacy, and participation in external meetings such as System of Care meetings.
  • Participate in case reviews and supervision, and attend regularly scheduled team meetings and trainings.
  • Maintain up-to-date case records for each client family, including weekly contact reports, service agreements, evaluations, and termination summaries.
Desired Qualifications
  • Home visiting experience is highly preferred.
