Requirements
  • Strong empathy, communication, and patient-centered care skills.
  • A Bachelor of Science in Nursing is required.
  • An active Registered Nurse license in good standing, with the ability to obtain additional licensure if needed.
  • Certified Diabetes Care and Education Specialist certification, or willingness to pursue it if not currently certified.

Responsibilities
  • Deliver patient-centered health coaching that integrates medical needs with patient preferences.
  • Triage patient-reported symptoms and side effects according to internal protocols, escalating to providers when appropriate.
  • Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight-management care and education via chat, phone, and video.
  • Coordinate care between patients, providers, support teams, and cross-functional clinical team members.
  • Create consults that align with internal protocols for providers.
  • Document patient care accurately and consistently to meet internal standards.

9amHealth provides virtual cardiometabolic care to prevent and manage chronic conditions like diabetes, obesity, high cholesterol, and hypertension. The service uses a B2B2C model with employers, health plans, and PBMs, and also offers a direct-to-consumer subscription. Its platform delivers personalized treatment plans, access to endocrinologists and pharmacists, at-home lab testing, connected devices, and prescription delivery, guided by step-therapy from lifestyle changes and generics to GLP-1s when needed. 9amHealth raises $26M to expand AI-driven virtual care beyond cardiometabolic conditions

9amHealth, a virtual specialty care platform, has raised $26 million in Series B funding led by Define Ventures, with participation from SemperVirens VC, Catalio Capital Management and NewHealth Ventures. The company says it has generated over $50 million in projected medical and pharmacy savings across two Fortune 100 employers. Founded in 2021, 9amHealth currently focuses on cardiometabolic conditions including obesity, diabetes, hypertension and hyperlipidemia. The platform combines clinical protocols, laboratory testing, medication management and continuous care delivery. The funding will support expansion into additional high-cost chronic conditions, which the company says account for up to 70% of employer pharmacy spend. 9amHealth uses AI-enabled clinical workflows to manage complex conditions at scale, partnering with employers, health plans and pharmacy benefit managers.
