Requirements
  • An Associate's Degree in Nursing is required.
  • At least 1 year of experience is required.
  • Proficiency with computers, the Internet, Microsoft Office, relevant software programs, and basic office and nursing equipment is required.
  • Successful completion of a state-approved pre-licensure nursing education program is required.
  • An active nursing license conferred by the state Board of Nursing is required.
  • The employee must obtain and maintain mandatory state and federal requirements and certifications for practice or occupation.
  • Current CPR certification is required.
  • Special care and dementia care employees must complete additional training in caring for and treating residents with dementia.
  • The employee must be able to work with elderly residents, families, and other health care staff members.
  • The employee must participate in Continuing Education Ohio State Requirements.
  • An active registered nurse license conferred by the Board of Nursing is required.
  • The employee must meet the residents' total care, supervisory, and emotional needs as determined by resident assessments.
  • The employee must monitor residents with increased emotional needs or behaviors that cause problems for themselves or other residents beyond regular supervision.
  • The employee must assist with prompt evacuation of non-ambulatory residents when necessary.
  • First-aid training documented by evidence is required.
Responsibilities
  • Oversee staff development, including training and education of nursing staff.
  • Ensure competencies for State Tested Nursing Assistants, including catheter care, medication administration pathways, and dressing changes.
  • Conduct monthly State Tested Nursing Assistant meetings to discuss new issues, training, and resident concerns.
  • Complete daily electronic medical record audits and follow up with employees to ensure accuracy.
  • Complete audits of the electronic medication program.
  • Liaise with the corporate office regarding electronic medical records and troubleshoot as necessary.
  • Assist the Director of Nursing as necessary.
  • Make weekly rounds with the physician and nurse practitioner.
  • Oversee the restorative care program.
  • Conduct interviews for new hires.
  • Review employee call-offs, call in off-duty employees, or adjust assignment sheets to provide adequate staffing coverage.
  • Direct and assign nursing staff.
  • Communicate appropriate information to team members, physicians, families, and the supervisor.
  • Assign resident care responsibilities and supervise subordinates on scheduled shifts.
  • Discipline subordinates through verbal or written warnings, or suspend them subject to discharge pending investigation.
  • Staff the department with capable employees.
  • Plan employees' work with regard to performance and skills.
  • Ensure employees are oriented and trained to perform their duties.
  • Set individual objectives and job expectations and ensure employees understand evaluation standards.
  • Provide guidance and supervision and periodically review employee performance.
  • Recommend employee promotions and pay when merited.
  • Coach, counsel, and discipline personnel as necessary.
  • Establish and maintain effective communication regarding performance with employees and leadership.
  • Manage employee personnel information.
  • Provide training and instructions on safe work practices and review work activities, materials, and facilities to eliminate potential hazards.
  • Maintain accountability for security within assigned areas of responsibility and take appropriate action when security infractions occur.
  • Receive, investigate, and respond to employee grievances.
  • Ensure subordinates are helpful, caring, and responsive to residents, families, employees, and other guests.
  • Oversee nasogastric and feeding-tube insertion, placement, irrigation, and related care.
  • Oversee colostomy care, including irrigation.
  • Oversee Foley catheter care, including insertion and irrigation.
  • Oversee Texas catheter application and straight catheterization.
  • Oversee tube feeding, including pump operation, intake and output calculations, resident weight monitoring, and residual tube placement.
  • Perform gastrointestinal, genitourinary, pulmonary, cardiac, circulatory, neurological, musculoskeletal, and integumentary assessments.
  • Perform positioning, transfers, and range-of-motion exercises.
  • Perform emergency procedures, including oxygen administration, suctioning, cardiopulmonary resuscitation, and ambu-bag use.
  • Perform glucometer checks.
  • Administer intravenous therapy, including intravenous-site care, heparin-lock monitoring, intravenous-solution administration, infusion-rate calculation, and infusion-pump operation.
  • Perform suctioning, including tracheotomy and nasotracheal suctioning and tracheotomy care.
  • Provide gastrostomy care through irrigation and placement checks.
  • Apply sterile and isolation techniques as necessary.
  • Collect laboratory specimens.
  • Provide central-line care, including flushing, infusions, monitoring, tubing and bag changes, and medication mixing.
  • Provide hyperalimentation care, including lipid piggybacking, site care, obtaining orders, checking bag contents and labels against orders, and monitoring complications related to central lines, peripherally inserted central lines, and hyperalimentation.
  • Supervise and assist with meal service and consult with the dietician regarding special dietary needs.
  • Monitor residents with significant weight loss or gain and report changes to the physician and dietician.
  • Identify residents at risk for nutritional problems and implement preventive measures.
  • Check food brought by family members for compliance with residents' orders, dietary needs, and safety requirements.
  • Document medications, treatments, and as-needed medications according to policy.
  • Complete the daily 24-hour report.
  • Monitor residents' intake and output.
  • Participate in the monthly medication renewal process.
  • Complete forms used throughout residents' charts.
  • Document changes in residents' conditions, including daily charting on ill residents and follow-up charting for residents with infections, incidents, and new admissions.
  • Interpret policies and procedures for personnel, residents, visitors, and government agencies as required.
  • Ensure resident charge slips are completed and forwarded for processing.
  • Maintain daily census reports and submit them to the business office as applicable.
  • Complete medical forms, reports, evaluations, studies, and charting as necessary.
  • Record new or changed diet orders and forward information to the Dietary department.
  • Report discrepancies concerning physician orders, diet changes, charting errors, and related issues to the supervisor.
  • Assist with acute and long-term care plans.
  • Participate in care conferences as necessary.
  • Assist with the Minimum Data Set.
  • Evaluate the effectiveness of care interventions, identify problems and trends, and develop alternative interventions.
  • Conduct investigations related to alleged abuse when appropriate.
  • Ensure progress notes reflect care plans and that care approaches are followed.
  • Ensure nursing assistants know resident care plans and refer to them before providing care.
  • Conduct pre-admission assessments.
  • Oversee the admission process, including orientation, physician orders, nursing admission assessments, medications, supplies, diagnostic tests, and communication of admission information to nursing personnel.
  • Obtain reports from admitting facilities before or at admission.
  • Oversee the discharge process, including post-discharge planning, resident education, discharge paperwork, communication with nursing personnel, and transportation arrangements.
  • Oversee patient transfers and ensure admission, discharge, and transfer documentation is accurate.
  • Ensure staff delivers quality care and services according to organizational policies and procedures and report violations, abuse, or mistreatment to proper authorities.
  • Follow safety, security, communication, dementia-care, and treatment protocols in the Dementia Special Care Unit.
  • Maintain confidentiality of necessary information.
  • Handle unusual occurrences calmly and logically to maintain continuity of business and duties.
  • Participate in special activities such as safety committees and mentoring, orientation, and new-hire activities.
  • Attend quality-assurance and other required meetings.
  • Perform other assigned duties.
