- Assist the Wellness Director with completing the clinical sections of monthly wellness visits and Personalized Service Plans.
- Inform the Wellness Director or Executive Director of medication or service-level changes that may result in a higher tier level or potential move-out.
- Assist with obtaining monthly weights and vital signs for each resident before completing monthly wellness health updates.
- Provide direct resident care as needed.
- At least one year of experience working with elders or disabled individuals.
- Knowledge and experience in the assisted living industry and dementia care or skilled nursing.
