Requirements
  • A degree in Social Work combined with registration or licensing with the provincial regulatory college is required.
  • At least 2 years of experience as a practicing Social Worker is required.
  • Working knowledge of applicable legislation, including the Mental Health Act, Substitute Decisions Act, and Healthcare Consent Act, is required.
  • Strong oral and written communication skills are required to interact effectively with clients and families with varying abilities for understanding.
  • Critical-thinking and problem-solving skills are required.
  • Knowledge of applicable community resources for clients and families is required.
  • Current First Aid and Cardiopulmonary Resuscitation certification completed in Canada within the past 12 months is required.
  • A current, clear Vulnerable Position Screening is required.
Responsibilities
  • Assess, resolve, prevent, or lessen the impact of psychosocial, physical, and mental-health-related issues for clients and their families in residential or institutional settings.
  • Conduct client assessments related to financial, accommodation, legal, or mental-health barriers.
  • Assist clients in setting specific, measurable, achievable, relevant, and time-bound goals.
  • Design and implement client treatment plans using evidence-based information, best practices, and clinical expertise.
  • Work collaboratively with a multidisciplinary team that includes government agencies, healthcare workers, and community resources.
  • Advocate for clients and families dealing with mental-health or disabilities, chronic medical conditions, addictions, palliative care, and other situations.
  • Identify and connect clients and families with available community resources.
  • Provide health teaching to clients, families, and caregivers regarding the client’s medical condition.
  • Evaluate intervention effectiveness, client responses, and progress toward goals, and determine whether goals need to be modified.
  • Communicate findings, treatments, and progress to the healthcare team.
  • Maintain professional client records for encounters and care plans in compliance with company policy and regulatory standards.
  • Maintain current knowledge, skills, and attitudes to provide safe, efficient, and effective service within the scope of practice.
