  • Current Maine licensure in social work.
  • A bachelor's degree in social work from an accredited institution.
  • American Heart Association Basic Life Support certification.
  • A valid driver's license.
  • A minimum of two years of experience in the field of licensure.
  • Demonstrated knowledge of health, social service, and funding sources.
  • Experience counseling individuals and groups with behavioral and emotional disorders.
  • Ability to work independently with limited supervision within the scope of practice and license.
  • Ability to make independent decisions regarding counseling, education, or referral of patients.
  • Ability to maintain confidentiality.
  • Reliable personal vehicle or another mode of transportation.
  • Emotional maturity, resilience, and stability.
  • Knowledge of best-practice standards and ethics.
  • Ability to multitask, prioritize, and organize.
  • Ability to form collaborative, proactive, and patient-focused relationships to maximize client healthcare outcomes.
  • Demonstrated communication and leadership skills with healthcare team members.
  • Ability to sit, stand, and walk for eight-hour shifts.
  • Ability to carry 20 pounds.
  • Ability to communicate effectively verbally and in writing.
  • Ability to work in an environment with potential exposure to blood or body fluids.

  • Frequent use of visual display terminals and contact with third-party payers.
Responsibilities
  • Provide timely coordination of quality healthcare services to meet individuals' specific healthcare needs in a cost-effective manner.
  • Adapt to meeting community members at health centers, in their homes, or at other community sites.
  • Provide and coordinate care with patients and involved people to develop appropriate plans addressing medical, rehabilitative, and financial needs.
  • Support community members facing social determinants of health barriers.
  • Use motivational interviewing techniques, advocacy skills, and knowledge of mental health and community resources to overcome barriers.
  • Offer assistance from community organizations and other team members and provide care coordination as appropriate.
  • Apply social work methods to provide emotional and mental health support to individuals.
Desired Qualifications
  • Experience in community and hospital social work and discharge planning is preferred.
  • Motivational interviewing experience is preferred.
