Requirements
  • Hold a professional or academically qualified nursing credential at bachelor's level and/or an advanced nursing specialization in intensive care/anesthesia, gerontopsychiatry, or a comparable specialty.
  • Have several years of experience caring for patients with dementia and/or delirium.
  • Have an interest in dementia, delirium, and cognitive impairments in the hospital setting.
  • Have foundational knowledge of clinical assessment and care for patients with dementia or delirium and willingness to pursue further qualifications.
  • Communicate effectively and collaborate across disciplines.
  • Be able to convey training content clearly and sensitize staff.
  • Be willing to contribute actively to the development of care quality and prevention measures.
  • Work independently and in a structured manner and contribute effectively within a team.
  • Meet applicable proof requirements under the Infection Protection Act as a condition of employment.
Responsibilities
  • Conduct consultation activities, including supporting nursing-diagnostic assessments of patients with dementia, cognitive impairments, or delirium risk.
  • Contribute to planning and implementing individualized treatment and care strategies, including non-medication measures and medication recommendations in collaboration with the medical team.
  • Support and train relatives in dealing with dementia or delirium symptoms.
  • Participate in case discussions and case analyses and recommend delirium-preventive environmental measures on hospital wards.
  • Participate in quality-assurance activities through documentation, evaluation, and involvement in subprojects.
  • Actively shape cross-sector care for patients supported by the consultation service together with the Community Health Nurses at the PORT Center City location.
