- Contribute to planning and implementing individualized treatment and care strategies, including non-medication measures and medication recommendations in collaboration with the medical team.
- Support and train relatives in dealing with dementia or delirium symptoms.
- Participate in case discussions and case analyses and recommend delirium-preventive environmental measures on hospital wards.
- Participate in quality-assurance activities through documentation, evaluation, and involvement in subprojects.
- Actively shape cross-sector care for patients supported by the consultation service together with the Community Health Nurses at the PORT Center City location.
