Requirements
  • Prior experience passing medications and providing care in an assisted living or memory care setting.
  • Must be at least 18 years of age.
  • Must pass the Elderwood Medication Technician test.
  • Must be able to assist with and operate a wheelchair.
  • Must be able to read, write, speak, and understand English at an intermediate or more advanced level for workplace communication.
  • Must complete Resident Care Assistant training on Relias, including 22 modules and a score of at least 80% on post-tests within 21 days of the start date.
Responsibilities
  • Direct and assist with the safe and proper administration of medication to assigned residents.
  • Administer medications and treatments as ordered by a physician, using the five rights as applicable.
  • Manage the storage and distribution of controlled-substance medications, including proper placement, labeling, and use of facility and medication-room refrigerator storage.
  • Respond immediately to resident requests.
  • Complete timely and accurate documentation, including e-interact, change-in-condition, transfer, STOP and WATCH, and other required records.
  • Report unusual symptoms, changes in condition, daily needs, and resident progress to the Director of Nursing or licensed professional.
  • Contact physicians, nurse practitioners, or physician assistants for orders or updates regarding changes in condition, and update the resident representative or emergency contact.
  • Transfer residents for evaluation when indicated.
  • Participate in change-of-shift reports and update nursing personnel on the next shift about resident care needs.
  • Follow safety rules, standards of practice, and facility procedures to maintain the safety of staff, residents, and visitors.
  • Implement emergency procedures correctly and efficiently.
  • Assist residents with activities of daily living, including bathing, dressing, eating, exercise, housekeeping, meal preparation, grooming, toileting, routine skin care, laundering clothing, personal emergency response, and supervised use of medical supplies or equipment, positioning, and transferring.
  • Notify the supervisor of changes in residents’ conditions or needs.
  • Receive resident-status reports from the nurse before providing care.
  • Follow plans of care when providing daily living and personal care services.
  • Inform the nurse after providing personal care, weighing residents, and completing other assigned tasks.
  • Maintain resident-care standards by protecting resident rights, treating residents respectfully, reporting equipment repairs or unsafe conditions, and helping maintain an orderly environment.
  • Complete at least 12 hours of in-service programs annually.
  • Maintain confidentiality of resident information and privacy during job duties.
  • Assist with dining-room service for meals as needed.
  • Inform the responsible nurse and document duties performed before leaving for a break or at the end of a shift.
  • Attend facility, staff, and required in-service meetings.
  • Apply and use personal protective equipment as required.
  • Use the electronic timekeeping system as directed.
  • Follow the facility dress code, including wearing a name tag.
  • Follow policies and procedures for electronic devices, computers, and tablets.
  • Abide by the company code of conduct, compliance policies, and HIPAA policies.
  • Perform other duties assigned by the supervisor, management staff, or Administrator.
Desired Qualifications
  • Completion of a medication technician training program within the last 12 months.
  • Certification by an approved basic training program in home health aide services with written evidence of certification.
  • A high school diploma or equivalent.
  • Long-term care experience.
  • A certified nursing assistant or licensed nursing assistant credential if working in a skilled nursing facility.

Elderwood partners with Trocaire College to advance nursing careers of current employees. Scholarship program provides both financial support and mentorship. Trocaire College in partnership with Elderwood, announced the launch of the Elderwood-Trocaire iCARE Career Pathway Scholarship. This initiative represents a $150,000 annual investment by Elderwood to support up to ten employees at a time as they pursue a practical nursing (LPN), registered nursing (RN), or bachelor's degree in nursing (BSN) at Trocaire. "This new scholarship program is another opportunity for Trocaire and Elderwood to work together to help strengthen the local healthcare workforce," said Dr. Bassam Deeb, president, Trocaire College. "The demand for qualified nurses locally continues, and with Elderwood's commitment to investing in its employees, this program provides financial support, mentorship, and a clear path forward for those looking to advance in their healthcare careers." Beyond financial support, the partnership between Trocaire and Elderwood provides mentorship and guidance from experienced clinical and operational leaders, as well as helping the participants successfully navigate their education and transition into advanced nursing roles. Full-time and part-time academic program opportunities are available. Employees must continue working at least 15 hours per week while enrolled at Trocaire and commit to one-year of employment with Elderwood upon program completion. Elderwood acquires Weinberg Campus for $47M

Employees, whose one-year contract expired last month, have only received one-year extensions since Elderwood announced plans in 2017 to purchase the Weinberg Campus for $47 million.
